Requesting Funding

NEW in 2017!

All chapters/sections may submit ONE Funding Request Form with their 2016 Annual Report and request for a 2017 Management Grant (or a chapter/section application). This form can be used for all other requests for funding. OSA Student Chapters must have all three signatures from the Advisor as well as the acting President and Treasurer. OSA Local Sections must have both the acting President and Treasurer.

The forms must be accompanied by an accurate United States tax form for the account holder/organization who will receive the funds. Names must match the Funding Request Form exactly (W-8 or W-9 Forms). All forms must be typed and provided in high-quality scans.

U.S. Chapters and Sections

All U.S. Chapters and Sections are paid by check to a group bank account. Please note the following:
 
  • Student Chapters that do not yet have group bank accounts should contact their university's office for student organizations to obtain a bank account and an  Employer Identification Number (EIN) or Taxpayer Identification Number (TIN) which is required for the W-9 Form.
  • Local Sections must request an EIN number directly from the IRS, and they must register as a 501c3 to receive a group bank account.

When completing the Funding Request Form please remember:

  • It is very important that the 'Name of the Account Holder' exactly match the actual name of the account holder as it is listed with the bank as well as Line 1 of the W-9 Form that accompany your application.
  • Checks will be mailed to the address on the form unless otherwise noted in the care of or additional instructions.  
W-9 Form Instructions
  • An authorized signatory for the chapter/section's account should complete and sign the form.
  • Remember that the Name of Account Holder on the Funding Request Form must match the name provided in Line 1 of the form. Under no circumstance can this be an individual's name.
  • Do not provide a Social Security Number (SSN). The number provided must be the EIN or TIN associated with the group bank account.
  • Original signatures are required.  



Non-US Chapters/Sections with a Group Bank Account

All non-US chapters and sections are paid by wire transfer in U.S. dollars.

An application for any OSA Student Chapter or Local Section charter or funding opportunity requires a Funding Request Form. When completing the form, please remember:

  • It is very important that the 'Name of the Account Holder' exactly match the actual name of the account holder as it is listed with the bank as well as the tax forms that accompany your application.
  •  Intermediary banks for wire transfer payments are required in some cases; make sure you are familiar with the bank's policies before requesting funding.
  • Since the chapter/section has a group bank account, the 'Additional Authorization' section is not required.
  • If your chapter or section has a group bank account to receive the funds, it must complete a W-8 EXP Form. Note: this form should be completed by an authorized signatory for the group bank account.
 
 
W-8 EXP Form Instructions

Please only complete the information below. Other fields can be left blank.

Part I, Line 1 - Name of the organization:

Enter the name of the account holder for the chapter or section's bank account. It must match 'Name of Account Holder' on the Funding Request Form.

Part I, Line 2 - Country of incorporation/organization

Enter the name of the country.

Part I, Line 3, Type of entity

Check the box for 'foreign tax-exempt organization'

Part I, Line 4: Chapter 4 Status (FATCA status):

Check the box for '501(c) organization. Complete Part III.'

Part I, Line 5 - Permanent Address

Enter the account holding organization's address. If the account belongs to your university, please enter the school's address.

Part I, Line 6 - Mailing Address

If your chapter or section's mailing address is different than the account holder's permanent address, you should enter it here. (Optional)

Part III, Line 19 - Qualification for Chapter 4 status

Check the box and certify that the beneficial owner (account holder) is not an insurance company.

Part IV - Certification

Sign, print your name and date the form. The person signing should be an authorized signatory of the group bank account and check the box underneath the signature.

 
 

Non-US Chapters/Sections without a Group Bank Account

  • It is very important that the 'Name of the Account Holder' exactly match the actual name of the account holder as it is listed with the bank as well as the tax forms that accompany your application.
  •  Intermediary banks for wire transfer payments are required in some cases; make sure you are familiar with the bank's policies before requesting funding.
  • Since the funding will be paid to an individual's bank account, the 'Additional Authorization' section is required.
  • If your chapter or section has authorized an individual to receive the funds, it must complete a W-8 BEN Form.

Part I, Line 1 - Name of Individual who is the Beneficial Owner

This should be name of the account holder who is authorized to receive the funds. It must match 'Name of Account Holder' on the Wire Request Form.

Part I, Line 2 - Country of Citizenship

Enter your country of citizenship.

Part I, Line 3 - Permanent residence

Enter your primary address.

Part I, Line 4 - Mailing address

Enter the chapter or section's mailing address or your mailing address if it is different from the Permanent Residence. (Optional)

Part III -  Certification

Sign, print your name and date the form.
