2) Mail or Fax: Complete and return the member enrollment form. Note: PDFs require Adobe Acrobat Reader.
New! You also have the option to pay your membership dues using your personal PayPal account. Simply download and complete OSA’s PayPal Membership Enrollment Form using Adobe Reader or Adobe Professional. The PayPal form is also available in Chinese.

Payment is due upon receipt and may be made by credit card, check, money order or bank transfer. Please make checks payable to the Optical Society of America. Payments must be made in U.S. dollars. Bank fees are the responsibility of the member. Dues include a non-deductable allocation of $9.69 for Physics Today and $10.00 for Optics & Photonics News.
