Executive Forum online registration is now closed. To register, download the Registration Form below and either fax (+1 202.416.1408) or email the completed form. You can also register on-site at the San Diego Convention Center at the Registration Counter located outside Room 28 (on the second level of the Convention Center).

Registration Fees

The registration fee for the Executive Forum includes a day of programming, a networking reception, breakfast, lunch and refreshment breaks the day of the program, and an exhibit only badge to OFC/NFOEC.

The OSA Corporate Member registration rate applies to all employees of Corporate Member companies, regardless of individual OSA member status.

*OSA Corporate Members benefit from a two-for-one registration package and non-members can take advantage of a special colleague registration fee. Colleagues must register at the same time to benefit from the special rates.


Checks and Money Orders

All checks must be in US dollars and drawn on a US bank. Make payable to the Optical Society of America. Please indicate the check number on the registration form.


Bank Drafts and Wire Transfers

Please indicate on your registration form which bank was used and when the deposit was made. Also, please notify OSA of the transfer to avoid any lost or unidentified payments. The wire transfer should include the attendee’s name or member’s name and a notation of Executive Forum Registration. Fax any support documents to Henry Cortez, Optical Society of America, fax number +1 202.416.1450 or email accrec@osa.org for more additional questions. Please incorporate any bank fees associated with your wire transfer as the bank may deduct their fees from the total sent to us. The remitter is responsible for these fees.


Refund Policy for Registration

A US$ 50 service charge will be assessed for processing refunds. A letter requesting the refund should state the registrant's name and the amount of payment and should be faxed to +1 202.416.1408. Requests for refunds that are received by MARCH 15, 2010, will be honored.

NO REQUESTS FOR REFUNDS WILL BE CONSIDERED AFTER MARCH 15, 2010.


Housing Information

Housing Deadline: February 19, 2010

A special room block is available to Executive Forum attendees at the Omni San Diego Hotel, which is the closest location to the Forum at the Convention Center. A limited number of rooms are available. Executive Forum attendees can also take advantage of the larger OFC/NFOEC housing block.
