You simply tell the Health Insurance Mart representatives your needs; they will match those needs to the plans they work with in your community, and provide the chance to choose the coverage that is right for you and your family. The Health Insurance Mart offers three basic types of insurance products: major medical (traditional), preferred provider option (PPO) and medical savings account (MSA).

The insurance companies they work with are all rated excellent by A.M. Best and the initial premium rates will remain the same with most carriers for at least one year. All this, combined with outstanding personalized service, makes the Marsh Affinity Group Services Health Insurance Mart an excellent resource.

Disability Income Plan
• Underwritten By UNUM Life Ins. Co.
• This Plan protects monthly income while disabled and unable to work. Provides up to $5,000 in monthly benefit in $100 increments. Members have the choice between a Career (up to age 65) and 5-year plan. The waiting periods range from 30 days to 180 days. The Plan offers a Partial and Residual Benefit Option.
• Not available in OR and WA.

Term Life Insurance
• Underwritten by New York Life Ins. Co.
• Provides practical, low cost coverage for members and their families. Coverage is available in amounts from $15,000 to $660,000. Every member under age 70 is eligible in addition to their lawful spouse and dependent children (up to age 24 if a full-time student). Coverage may remain in force up to age 80. Policy contains an Accelerated Death Benefit and a Waiver of Premium Benefit enhancement.

Cancer Expense
• Underwritten by Monumental Life Ins. Co.
• This plan can protect you and your family from the high costs associated with cancer treatment. Benefits are paid directly to you, regardless of any other coverage you may have. The Plan offers standard and high option benefit options.
