Exhibitors: Exhibitor Registration is not available online. Please complete the registration form below and fax or mail it in.

Registration implies consent that any picture taken during OSA sponsored events can be used for meeting and promotional purposes without remuneration and that your name will be placed on an attendee list which is distributed to BIOMED/DH meeting attendees.

Full Conference Registration
The registration fee for BIOMED/DH 2010 includes admission to the Technical Sessions, Exhibit, Coffee Breaks, the Conference Reception and one copy of the Conference Program and Technical Digest on CD-ROM.

Student Registration
Full-time students (graduate and undergraduate) are entitled to the same privileges as regular registrants. Students are required to present valid student identification when they pickup their registration materials.

Emeritus Registration
Emeritus members may also register at the discounted rate. To qualify for emeritus status, members must be fully retired and must have been an OSA member for at least 10 years. The member's age plus the number of years of membership in OSA must equal 75 years or more.

Exhibit Only Registration
Exhibit Only registration allows attendees to access only the exhibit hall. This registration does NOT include access to the Technical Sessions, Conference Banquet or a Program with Technical Digest on CD-Rom. To attend any of the technical sessions or receive the Program, you must register for a Technical Registration.

Special Items for Purchase

One Conference Reception ticket is included in the Full Technical Fee. Guest tickets may be purchased for US $50 per person. The Conference Reception will be held Monday, April 12, 2010 in the evening.

Extra copies of the BIOMED/DH 2010 Program and Technical Digest on CD-ROM are available for purchase for US $75.

Contribute to the Jean Bennett Memorial Travel Grant Endowment
We ask that you donate to help establish a student travel grant in memory of Jean M. Bennett, a highly decorated research physicist who was recognized for her contributions to the studies of optical surfaces, and served as OSA’s first female president. With your help we will present the first travel grant in 2010. This program will recognize the research excellence of a student presenting their work at FiO. Please join us in supporting this program through a donation of $25. If you would like to provide an additional contribution, please visit www.osa-foundation.org/give.

Important Information

Checks and Money Orders
All checks must be in US dollars and made payable to the Optical Society of America. Please indicate the check number on the registration form.

Bank Drafts and Wire Transfers
Please indicate on your registration form which bank was used and when the deposit was made. Also, please notify OSA of the transfer to avoid any lost or unidentified payments.

Refund Policy for Registration
A US $75 service charge will be assessed for processing refunds. A letter requesting the refund should state the registrant's name and the amount of payment and should be faxed to +1 202.416.6140. Requests for refunds that are received by April 2, 2010, will be honored. All refund requests must be made in writing.

NO REQUESTS FOR REFUNDS WILL BE CONSIDERED AFTER April 2, 2010.
