Registration Fees
The registration fee for the workshop includes a day of programming, a networking reception, breakfast, lunch, and refreshment breaks the day of the program.The member registration rate applies only to employees of OSA Industry Development Associate (OIDA) Member companies. Checks and Money Orders
All checks must be in US dollars and drawn on a US bank. Make payable to The Optical Society. Please indicate the check number on the registration form.Bank Drafts and Wire Transfers
Please indicate on your registration form which bank was used and when the deposit was made. Also, please notify OSA of the transfer to avoid any lost or unidentified payments. The wire transfer should include the attendee’s name or member’s name and a notation of Executive Forum Registration. Please incorporate any bank fees associated with your wire transfer as the bank may deduct their fees from the total sent to us. A US$ 75 service charge will be assessed for processing refunds. A letter requesting the refund should state the registrant's name and the amount of payment and should be faxed to 202.416.1408. Requests for refunds that are received by 13 MARCH 2017, will be honored.NO REQUESTS FOR REFUNDS WILL BE CONSIDERED AFTER 13 MARCH 2017.
