Fellow Nomination Reference Forms

If you have been asked to complete a Fellow Reference Form for a nominee, you may use the online reference form below or download and mail the PDF form.

References are asked to describe their specific knowledge of the nominee's professional and other accomplishments that support election to OSA Fellow.  In addition to completing the form, may also cut  and paste text or upload a document into the on-line form.  If you are not using the on-line reference form, you can attach a letter or comments and submit with your completed form.

Directions for submitting your nomination reference

Enter the Nominee's and Nominator's names in the fields below and click the Next button.

You must then complete the online submission form that includes contact information for the referee, privileged information and ratings.

Navigating the Submission Screens

On each of the screens, enter the appropriate information into each field and click the Next button to proceed to the next screen. Required fields are indicated with an asterisk (*). Clicking the Next button will save your entries as you move from screen to screen.

You may click the Previous button to return to the preceding screen to view or update the information already entered. Once you click the Next button, your changes will be saved. However, it is not possible to save a partially complete form and return to it later to complete.

After completing the Submission Form and uploading your file, the system will generate an email to you with a summary of the submission form information. The email will be sent to the email address you provided on the Referee Information screen. Additionally, the last screen provides a read-only summary of the submission form.
