Requesting Chapter/Section Funds

Receiving funding for your student chapter or local section is an easy process! If you have been awarded a Youth Education or Activity Grant or you are a newly established chapter seeking to claim the $250.00 USD start-up funds, carefully read and follow the directions below and the directions on the check or wire request. All funds are disbursed in U.S. dollars.
 

Chapters/sections outside the U.S.:

An officer of the chapter or section must complete the W-8 Form. Complete your name and address. Sign the form; an original signature is required. Please review the W-8 Instructions at the bottom of this page.

3) Submit the forms:

  • Youth Education and Activity Grants forms must be submitted through the OSA Grants Database. Your login credentials were emailed to you. If you have not received log-in information, please contact us at chaptersandsections@osa.org. Please do not create a new organizational profile as it will not allow you to submit the forms.
  • Start-up Funds forms should be scanned and emailed to chaptersandsections@osa.org.


U.S. chapters/sections:

  • An officer of the chapter/section must complete the W-9 Form. Complete your name, address, social security number and chapter/section EIN. Sign the form; an original signature is required.
  • U.S.-based chapters/sections must provide OSA with an Employer Identification Number (EIN) to receive funding. New chapters and sections may obtain an EIN at the Internal Revenue Service website.
  • U.S.-based chapters/sections must have a bank account in the name of the organization; for example, the “OSA Student Chapter at Your University.” U.S. chapter/section checks cannot be written to individuals.

*$250 start-up funds are distributed to chapters once within the first year of establishment. If you have any questions regarding whether your chapter is eligible to receive these funds, please email chaptersandsections@osa.org.
 

 W-8 Form Instructions:

  • Part I, Line 1 – Name of Beneficial Owner

Usually, the “beneficial owner” is the same as the name of the individual or entity to whom OSA is making a payment (you). The only time the beneficial owner would be different would be if the person/entity to whom OSA is making a payment will simply obtain the funds and pass them on to another individual or entity (for example, if the payment is made to an individual, who will transfer the funds to an organization, then that organization is the beneficial owner).

  • Part I, Line 2 – Country of Incorporation

If the OSA payment is going to an individual, then mark this as “N/A.” If the OSA payment is going to an organization or corporation, then fill in the name of the country in which the organization or corporation is registered.

  • Part I, Line 3 – Type of Beneficial Owner

If the amount is to be paid to an individual, check the “individual” box. If the payment is going to an organization or chapter, the box you check is what corresponds to how your organization would be treated under U.S. tax law. So, if the payment is going to a university, check the “Tax-Exempt Organization” box, since that is how these organizations are classified in the U.S.

If the amount is being paid to a chapter, then the box you check depends on whether the chapter is a separate organization, which normally means that it has its own bank account (separate from an individual’s account or a university’s account). If the chapter has its own account and is considered a separate entity, you should check the “corporation” box if the chapter has been incorporated, or the “tax-exempt organization” box if the chapter has its own bank account but is not formally incorporated.

  • Part II, Line 9 – Claim of Treaty Benefits

If the payment is being made to an individual, check box 9a and fill in your country of residence.

If you have either a U.S.-issued Social Security Number (for individuals) or a U.S.-issued Employer Identification Number (for corporations and organizations), check box 9b and fill the number in on line 6. You are generally not required to obtain a SSN or EIN by merely receiving a payment from OSA.

If the payment is being made to a corporation or tax-exempt organization, then check box 9c.

Boxes 9d and 9f should never be checked.

  • Part II, Line 10 – Special Rates and Conditions

This line does not to be completed in connection with the types of payments you receive from OSA.

  • Part III, Line 11 – Notional Principal Contracts

This box should never be checked in connection with payments from OSA.
