Requesting Chapter/Section Funds

Receiving funding for your student chapter or local section is an easy process! If you have been awarded a Youth Education or Activity Grant or if you are a newly established chapter seeking to claim the $250.00 USD start-up funds, carefully read and follow the directions below and the directions on the check or wire request. All funds are disbursed in U.S. dollars.
 

Chapters/sections outside the U.S.:

1) Complete a funding request form:

2) Complete a W-8 Form:

  • An officer of the chapter or section must complete the W-8 Form. Complete your name and address. Sign the form; an original signature is required. Please review the W-8 Instructions at the bottom of this page.
  • If your chapter or section has authorized an individual to receive the funds, it must complete a W-8 BEN Form. Please review the W-8 BEN Form instructions below.
  • If your chapter or section has group bank account or is using a university bank account, it must complete a W-8 EXP Form. Please review the W-8 EXP Form instructions below.
  • An original signature is required on all forms.

3) Submit the forms:

  • Youth Education and Activity Grants forms must be submitted through the OSA Grants Database. Your log-in credentials were emailed to you. If you have not received log-in information, please contact us at chaptersandsections@osa.org. Please do not create a new organizational profile as it will not allow you to submit the forms.
  • Start-up Funds forms should be scanned and emailed to chaptersandsections@osa.org.


U.S. chapters/sections:

  • An officer of the chapter/section must complete the W-9 Form. Complete your name, address, and chapter/section EIN. Sign the form; an original signature is required.
  • U.S.-based chapters/sections must have a bank account in the name of the organization; for example, the “OSA Student Chapter at Your University.” U.S. chapter/section checks cannot be written to individuals.
  • U.S.-based chapters/sections must provide OSA with an Employer Identification Number (EIN) to receive funding. New chapters should contact their university's center for student groups and activities to see if they can use a university account. If a chapter is unable to obtain an EIN through this route, please contact chaptersandsections@osa.org.

*$250 start-up funds are distributed to chapters once within the first year of establishment. If you have any questions regarding whether your chapter is eligible to receive these funds, please email chaptersandsections@osa.org.
 

 W-8 BEN Form Instructions:

Please only complete the information below. Other fields can be left blank.

Part I, Line 1 - Name of Individual who is the Beneficial Owner

This should be name of the account holder who is authorized to receive the funds. It must match 'Name of Account Holder' on the Wire Request Form.

Part I, Line 2 - Country of Citizenship

Enter your country of citizenship.

Part I, Line 3 - Permanent residence

Enter your primary address.

Part I, Line 4 - Mailing address

Enter the chapter or section's mailing address or your mailing address if it is different from the Permanent Residence. (Optional)

Part III -  Certification

Sign and date the form.

W-8 EXP Form Instructions:

Please only complete the information below. Other fields can be left blank.

Part I, Line 1 - Name of the organization:

Enter the name of the account holder for the chapter or section's bank account.It must match 'Name of Account Holder' on the Wire Request Form.

Part I, Line 2 - Country of incorporation/organization

Enter the name of the country.

Part I, Line 3, Type of entity

Check the box for 'foreign tax-exempt organization'

Part I, Line 4: Chapter 4 Status (FATCA status):

Check the box for '501(c) organization. Complete Part III.'

Part I, Line 5 - Permanent Address

Enter the account holding organization's address. If the account belongs to your university, please enter the school's address.

Part I, Line 6 - Mailing Address

If your chapter or section's mailing address is different than the account holder's permanent address, you should enter it here. (Optional)

Part III, Line 19 - Qualification for Chapter 4 status

Check the box and certify that the beneficial owner (account holder) is not an insurance company.

Part IV - Certification

Sign and date the form.
