Fellow Nomination Forms
OSA bylaws specify that: "Any regular member who has served with distinction in the advancement of optics is eligible for transfer to the class of fellow."
You must be a current OSA Fellow member to nominate a regular OSA member to the rank of Fellow. To nominate, complete this Fellow Nomination Form and arrange for three to five individuals (at least one of whom must also be an OSA Fellow) to complete a Fellow Reference Form. Five is the maximum number of references that the committee will consider.
You may use the online form, download and mail the form below, or email awards@osa.org for Word files of the forms or printed copies. Supporting material from the nominator may not exceed 10 pages.
Nominations are reviewed by the Fellow Members Committee each summer. Their report must be approved by the Board of Directors. Elected Fellows are notified in November following the deadline. Fellows shall pay the same dues as regular members.

Online Fellow Nomination
Directions for submitting your nomination Select one of the OSA Technical Divisions that most closely fits the work of the nominee from the drop-down menu below.
You must then complete the online submission form that includes contact information for yourself, the nominee and at least three references. Additionally, you must upload a file (of not more than 10 pages) that includes the nominee’s academic record and honors; areas of professional specialization; other noteworthy accomplishments in science, engineering and public service; principal professional society memberships and activities; employment history and publications and patents.
The following provides general guidelines for navigating the submission screens.
Navigating the Submission Screens On each of the screens, enter the appropriate information into each field and click the Next button to proceed to the next screen. Required fields are indicated with an asterisk (*). Clicking the Next button will save your entries as you move from screen to screen.
You may click the Previous button to return to the preceding screen to view or update the information already entered. Once you click the Next button, your changes will be saved.
There is no specific time limit in which you need to complete the submission process. As long as you do not close the submission form in the browser, the information on the saved screens will be available and you can return later and complete the unfinished portion of the submission and submit. If you close the submission form, the information will not be saved.
After completing the Submission Form and uploading your file, the system will generate an email to you with a summary of the submission form information. The email will be sent to the email address you provided on the Your Information screen. Additionally, the last screen provides a read-only summary of the submission form.
