Proxy Server Questions

Please answer the following questions regarding your use of proxy servers. Please contact your computer services department for assistance.


  1. Are these Proxys for the sole use of the facility named on the agreement? Please indicate other locations within the your Corporation the Proxys will also service.



  2. Please indicate if the registered Institution is the only Institution using the IP addresses (including those of proxy/caching servers) submitted to OSA for enabling access to OpticsInfoBase.



  3. Please indicate and identify any of the IP addresses registered by the Institution which are for servers that allow remote users to log in or are accessible by users outside the subnets registered. Please describe the type of remote access that is provided.



  4. Please provide the IP address of the proxy/caching server(s) used by your Institution. A proxy server is a computer used to forward browser request from an organization's internal network to the Internet, and to pass replies from the Internet back to the organization's internal network. A proxy server can also cache frequently requested Web pages.



  5. If your Institution employs a proxy/caching server, please indicate how your Institution will ensure that the Terms and Conditions as specified in our Subscriber Agreement are adhered to. In particular, how your Institution will limit access to "authorized users" as defined in the agreement.
